Client Agreement – Code of Conduct

1. The client will consistently receive respectful treatment.
2. Information shared during the Initial Consultation and subsequent sessions will be kept confidential.
3. The hypnotherapist has a professional obligation to report to relevant authorities any concerns they believe where the client may intend to harm themselves, the therapist or others.
4. If the client is receiving medical treatment then a diagnosis must be sought when possible, and health professionals may be informed of hypnotherapy treatment.
5. The Association for Solution Focused Hypnotherapy Code of Practice is readily accessible for reference.

Consent for Treatment

1. The therapist has thoroughly explained procedures and treatment details.
2. I understand that regular listening to the track each night is a crucial aspect of the treatment.
3. Recognizing that the success of treatment is influenced by the commitment to desired changes and adherence to session formats.
4. A respectful relationship will be maintained between the client and therapist.

Cancellation Policy

1. Due to a busy practice and a waiting list, considerate cancellation with at least 48 hours’ notice is requested.
2. The full appointment fee will be charged if less than 24 hours’ notice is given for cancellation.
